    Times are limited to 16 people testing!

    Please fill in all the information below SEPARATELY for each student testing.
    Please Choose the next 2 options carefully! We want to make sure you order the right size and colors!!!!

    5:00 - BBT Testing (16 spots)
    6:00 - BBT Testing (16 spots)
    7:00 - Masters Club Testing (Blue and Above)
    Please FILL this in and send ASAP. I will send an invoice from Square, By Clicking the box below, you are giving consent for your child to test at the above top right Date/Semester/Date/Time. 
    Testing Fees
    Color Belts - $70+gst - Paid Early $60+gst
    Black Belt Midterms - $70+gst
    Black Belt Rank Test - $250+gst minus Belt Deposit
    ***Black Belt on the wall - $50 Non Refundable Deposit
